At Easterseals, our Case Management Services for individuals who are Deaf, DeafBlind, or hard of hearing are designed to remove barriers, connect people with the resources they need, and promote independence at every stage of life.
Our case managers are fluent in American Sign Language (ASL) and have received training in Deaf culture, ensuring that communication is accessible and culturally respectful. We work closely with individuals and families to create personalized service plans based on their unique goals, challenges, and needs.
What We Offer:
- Individualized Service Planning
We assess each person’s strengths and needs to develop a customized plan for reaching personal, vocational, and independent living goals. - Resource Navigation & Referrals
Assistance accessing community resources, including:- Social Security benefits
- Public housing and utility assistance
- Medicaid/Medicare or health insurance
- Food programs (SNAP, WIC)
- Assistive technology and communication devices
- Interpreter and Communication Support
Help coordinating ASL interpreters, video relay services (VRS), captioning, and other tools to support access in healthcare, legal, and community settings. - Advocacy
Support with self-advocacy and representation during meetings with schools, employers, government agencies, and medical providers to ensure communication access and equal treatment under the law. - Collaboration with Other Services
We partner with other community providers to provide comprehensive, wraparound support. - Support for Families
Guidance for parents or caregivers of Deaf children or adults in understanding systems, services, and how to advocate effectively.

Our Case Management Services are available to individuals who are Deaf, DeafBlind, or hard of hearing of all ages. Services are tailored to each person’s language preference and communication style.
